You will now be taken to the ‘Questionnaire’ tab. The question/control box will appear on the right-hand side. In the relevant (and only) bar (that says 'Enter question'), type the control/question you want to add to the assessment; for example, ‘Do you perform regular tests on your cyber security system?’.

After typing the question, select either the ‘CREATE NEW ANSWER’ or the ‘OPEN ANSWER BANK’ options.

If you choose the ‘CREATE NEW ANSWER’ option, a dropdown menu will appear containing eight 'Answer type' options (‘Single line text’, ‘Multi line text’, ‘Radio button’, ‘Checkbox’, ‘Drop down’, ‘Multi select’, ‘File upload’, and ‘Date picker’, and 'Numbers Only'). Select the most appropriate 'Answer type' for the question/control (i.e. Select the 'Answer type' that will enable the assessee to answer the control/question in the most efficient and applicable way.)

Once you created the control, click the 'DONE' button.
